Date and Place of Birth Proof : Birth Certificate (issued by a Municipal Authority), School or College leaving Certificate, S.S.C Board Certificate.

Qualification Proof : Degree Certificates , 1st, 2nd, 3rd Years Mark sheets, Passing certificate.

Tax Payee : Form 16.(Salary deduction Certificate for 3 years, & Tax Return Challans) 3 Yrs.Self- Assessment Tax paid challans, Assessment orders and Appendix "A" tax clearance certificates (with PAN card).

No Objection Certificate : Required in case of Employees of Govt. / Public Sector / Statutory body.

Marriage Proof : Marriage certificate issued by a "Registrar of Marriage", Or Marriage photo, Invitation card,& Marriage Affidavit as per format GIVEN BY Passport Office and
also assisted by Metropolitan magistrate only (Notary not allowed).
